Self-Pay Fees

$175 per session

Accepted Insurance

In-network with Blue Cross Blue Shield of Illinois PPO and Northwestern Student Health Insurance (NU-SHIP). Your copay/co-insurance will be due at the end of each session and I’ll submit claims for you. It’s important to be aware that when you utilize insurance, your diagnosis and dates of services become part of your health insurance record.

Out-of-Network Plans

If you carry insurance with another company, the full session fee will be due at the end of each session. Most insurance plans will cover a portion of session fees as “out-of-network” so I encourage you to call your insurance company to learn about your coverage. I’ll provide you an invoice which you can submit for reimbursement.

Payment

Payment is due at the end of each session via credit card, cash, or check.

Cancellation Policy

Appointments must be cancelled or rescheduled at least 24 hours in advance of the scheduled session, otherwise you’ll be charged the full fee of $175.

Complimentary Phone Consultation

I offer a 15-minute phone consultation so you can learn more about my approach and determine if we’re a good fit. If you decide you’d like to move forward, we’ll schedule an initial appointment.

Good Faith Estimate

  • Under the law, healthcare providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment and hospital fees.

  • Make sure your healthcare provider gives you a Good Faith Estimate in writing at least one business day before your medical service or item. You can also ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate.
